Very Low Calorie Diet Therapy in Obese Patients

Abstract
Very low calorie diet(VLCD) is defined as diet which contains less than 600 calories. This therapy was developed as a replacement of total starvation which might be the most efficient for caloric restriction but dangerous for health. This therapy ois usually provided as formula diet. When Sandz company introduced this diet as "Modifast" or "Optifast" they asked us to examine effectiveness and safety. We organized muticenter trial to meet their requests. We found that average body weight reduction was 7.9 kg during 4 weeks and no severe adverse effects, although many marginal ones were- observed. We also found that this therapy reduced hyperglycemia, hyperlipidemia and hypertension. In our study, exaggerated insulin secretion in severe obese patients was reversed by VLCD therapy. We also recognized that we should give careful consideration into indication, compliance, utilization period, as well as adverse effects. The most important problem is the maintenance of reduced body weight after VLCD therapy. In summery, VLCD is useful for body weight reduction, improvement of abnormalities associated with obesity, however, some cautions are necessary for utilizing this therapy.
